A Turkish viewer-factory DICOM app: on medical grossing charts in 40 countries, only China is actually paying
What it is A scan viewer that lets patients open the CT and MRI disc files their hospital gave them on an iPhone. Free to download; Pro is a weekly or annual subscription.
Why it took off It hasn't really taken off: people who have the scans but no computer search "DICOM" in the store, and impulse-subscribe when they find it. The "charting in dozens of countries" is the result of a low bar on small-country medical grossing charts. The only market with real volume is China, where the Chinese title is placed first.
CHow you could follow it Stop making DICOM apps. Learn the factory playbook behind it: find a specialist file format ordinary people can't open, lead the title with the format name in each language, register as the system's default opener, show a subscription prompt after the first file opens, and judge demand only by the big-country charts.

In 2026 October 13, someone on the China App Store gave "DICOM Medical Imaging Viewer - MesDICOM" one star, writing: "What is this junk, my free software works better than this, what did I buy!!?" That review tells two things: someone got a CT or MRI scan from a hospital, searched "DICOM" on their phone, found this app, paid on the spot, and then regretted it. Three weeks later, in November 5, the app reached #4 on Panama's medical grossing chart, yet it had zero ratings in Panama. This article covers what it is, how much of the chart numbers are real, where the money comes from, how crowded the space is, and whether you should follow it.
What it is: viewing your hospital scans on a phone
MesDICOM is a medical imaging viewer for iPhone and iPad. The first line of the China store page reads: "Open files, folders, ZIP archives, or individual DICOM files copied from an imaging disc to view exam images on your device." It covers the features fairly thoroughly: window width and level adjustment, 30 presets, multiplanar reconstruction in axial, coronal, and sagittal views, 3D volume rendering, measurement of distance, angle, and ROI, PDF export, and connection to DICOMweb servers. Images stay on the device, with no sign-up. The store page also clearly states that it is "not a medical device" and must not be used for diagnosis.
It's made by ONNEX YAZILIM in Eskişehir, Turkey, released in Aug 24, updated in Oct 16 to 1.2.5. The company has 12 apps under its name, mostly "viewers for some file type": WristDocs for reading PDFs on Apple Watch (399 ratings), Mes3D for viewing STL 3D models (released April 2026), and now one for DICOM. 3D rendering, file import, and the subscription paywall are all off-the-shelf. Swap the file format, add 13 languages, and you have a new app.
Free to download; Pro is a subscription: weekly $3.99 or $5.99, annual $19.99 or $29.99, lifetime $79.99.
What's behind the "40-country grossing chart" claim
From Oct 28 to Nov 6, it appeared on the charts of 31 to 41 countries each day, reaching the medical category charts of 65 storefronts in total. That sounds big, but checking the rating counts changes the picture. Its best rankings were Panama #4, Slovenia #4, Ecuador #6, Egypt #7, Iceland #13, and Singapore #20, and all of those storefronts had 0 ratings.
Panama's trend tells the story best. From 10-28 to 11-03, it hovered between #52 and #184 on the local free medical chart; on 11-05 it suddenly jumped to #4,11-06 and #8 on the medical grossing chart. A zero-rating app breaking into the top 5 overnight likely means a handful of subscriptions (inference). The bar for small-country medical grossing charts is nearly zero.
Strip out those storefronts and the volume is tiny: about 25 ratings worldwide in total, with 0 to 3 new ratings per day. The US storefront has 6 ratings and has never charted. The UK, Germany, and Japan only show up occasionally, around #63 to #196 on the medical grossing chart.
China is the only market with real volume. Ratings rose from 10-28 to 7 to 11-06 to 12, averaging 4.0 stars. It stayed on the medical grossing chart for 10 consecutive days, ranking between #35 and #86, mostly around #35 to #57. 11-06 It ranks #50, with "Piyo Log" ahead and "Discover the Human Body 3D" and AMBOSS behind it. In money terms, that's probably tens of dollars a day (estimate, unverified). The Google Play version launched in Sep 23, and as of 11-06 it had 1,331 installs.
Where users come from: search and "Open with"
We can't find any promotion of it anywhere: zero mentions on X in 90 days, no related TikTok videos in search, and no media or forum posts. It doesn't show up in the Meta, TikTok, or Google ad libraries (those cover only part of the market, so absence doesn't mean no spend; whether it runs Apple Search Ads is not visible from outside).
That leaves two routes, both inside the iPhone system:
- Store search. The China store name is simply "DICOM Medical Imaging Viewer," with the keyword at the front. Titles in 13 languages follow the same approach. People searching "DICOM" know exactly what they want: they have a scan and want to view it right now.
- File association. A patient copies the disc files to their phone, or gets a ZIP, and when they tap it iOS lists MesDICOM under "Open With". It even recognizes DICOM files with no extension. The mechanism is confirmed. How many people it brings in is unknown.
So the user is an anxious patient or family member. They just got the scans, have no computer nearby, and the hospital disc won't open on their device. They pay weekly for "see it right now", and once they've looked, they don't need it anymore. It's a one-time transaction with little retention. The only three written reviews in the China store are all one-star ("cumbersome", "not usable", plus the one at the top). The average rating is 4.0, which suggests people who only rate and don't write do like it more, but the sample is too small to show either way.
Who it's competing with
Search the App Store by name for DICOM, PACS, image viewer, medical imaging, and you'll find 100 apps. Only a few established ones have real scale. 2026 years on, a dozen or so newer ones have appeared:
| Product | Launched | Scale (11-06) | Compared with MesDICOM |
|---|---|---|---|
| IDV - IMAIOS DICOM Viewer (French medical education company) | 2018-12 | iOS about 3,100 ratings (China 815); Play 956k installs | The category leader. Similar features, with a brand and reputation |
| Falcon Mx / MD / Patients (UK) | From ¥2020-02 | Mx about 2,171 ratings, charted in 61 stores | Long-established pro version. October just split out a version for patients |
| Imaging anatomy atlas (Osmanco) | 2021-02 | 1,796 ratings in China | Built for medical students to learn anatomy, with DICOM viewing on the side |
| Ossidex (its China-store name is "DICOM阅片 CT 核磁共振 X光片") | 2026-04 | 84 ratings (China 26) | Almost the same playbook: indie developer, multilingual titles, subscription. Launched 4 months earlier, and it has more than 3 times MesDICOM's ratings |
| DICOM Viewer: MRI CT Studio, DICOM Vault (both from Turkish developers) | 2026-07 / 05 | 1–2 ratings | Same time, same playbook |
| DCM & MRI Reader, Slicora, Priors, Universal Dicom Viewer, DicomLens, and others | 2026-03 to 10 | From 0 to a few dozen ratings | A batch of small viewers that appeared in 2026, most with fewer features than it |
| MicroDicom, Horos, Weasis, RadiAnt | Years | Mainstream on desktop | Free. The TikTok videos teaching people "how to view hospital discs" recommend MicroDicom |
| Hospital imaging cloud, scan-to-view QR codes | — | Already widespread in many Chinese hospitals | Free, no disc needed, viewable directly in WeChat |
The last row matters most for the China store. More and more hospitals just hand out a QR code, and a phone scan opens the images, so discs are used less and less. MesDICOM's only paying market is shrinking in demand.
What it does better than others, and whether that's enough
Feature-wise it really is more complete than most 2026-year-old newcomers. It has MPR, 3D, and DICOMweb, and is close to IDV. But patients searching for "DICOM" can't tell these features apart from the store page. What actually decides their download is the title, the icon, and the ranking. Its real difference is distribution: localized titles in 13 languages, plus file association. Both can be learned in a weekend. Ossidex's Chinese name uses the same approach, and it did so earlier.
If it were you
The verdict: don't go after DICOM as a format. A dozen-plus players have already crowded in, and the leader has reputation. What's worth copying is the "file viewer factory" behind it: a specialized format, multilingual keyword titles, the system Open With menu, and a subscription wall shown on first open.
Who it fits: a small team that already has code for file import, rendering, and a subscription paywall, can ship a new app a month, and lives off long-tail store search.
Who it doesn't fit: anyone who wants to build a brand or long-term subscriptions from one app. These users leave once they've seen their scans, and refunds and bad reviews pile up. Medical review is also stricter. It's also not for building pro reading software for doctors. That belongs to OsiriX and Falcon, and it requires credentials and hospital channels.
How to do it: look for professional formats that ordinary people end up with but can't open. DICOM, STL, and EPUB are already taken, so look at DWG, GPX, specific camera RAW formats, and various engineering or survey files. Search the format name on the App Store and check whether the top ten are just one or two old apps with few ratings and no update in the past year. First make it open and display files correctly, start each localized title with the format name, register as a system Open With handler, and only show the paywall after the first successful open.
How to get users: skip social media. Rely on store search and multilingual titles, plus file association. To judge whether it's really making money, look only at large-country or high-barrier charts (for example, the China-store medical Top Grossing, places 35 to 60), and ignore rankings in small-country stores.
When to move: for DICOM alone, the window has already closed. The first half of 2026 was already crowded. Other formats have no clear deadline. But AI-assisted coding makes building a viewer cheaper every day, and once a format is claimed, later entrants can only fight over the leftover long tail.
What would prove this wrong: if by December MesDICOM drops out of the top 100 in the China-store medical Top Grossing and its ratings stay in the low dozens, it means even this China-store revenue is short-lived, and each app in the "viewer factory" earns too little to be worth building. Conversely, if it holds steady on the Top Grossing charts in large markets like the US or Japan, the approach is worth more than we estimated, and the grade should go up.

Why MesDICOM (DICOM Medical Image Viewer) appears on the medical Top Grossing charts in 30–40 countries
- App: DICOM Viewer - MesDICOM (China store name: "DICOM医学影像查看器 - MesDICOM"), apps.apple.com/app/id6796643734
- Developer: ONNEX YAZILIM ANONIM SIRKETI (Eskisehir, Turkey; website onnex.co; 12 apps on the App Store)
- Launched 2026-08-24; current version 1.2.5 (2026-10-16); Medical category; free download + Pro subscription (weekly $3.99/$5.99, yearly $19.99/$29.99, lifetime $79.99, from the store page checked against the recall card)

Bottom line
It didn't really "break out." It's another viewer built by a Turkish indie app factory using a repeatable playbook. The "30–40 country Top Grossing" claim is an illusion created by the very low bar for the Medical grossing chart in small countries: when it ranked #4 in Top Grossing in Panama, it had 0 ratings there. The only market with real volume is China, where it spent 10 days ranked between #35 and #60 in Medical Top Grossing, with roughly 25 total global ratings. Traffic mostly comes from App Store search for "DICOM", driven by localized titles in 13 languages, plus iOS file association with "Open in MesDICOM." It doesn't show up in ad libraries, on X, on TikTok, or in the press.
1. What the data actually shows
Ratings (download proxy)
- Total global ratings bounce between 20 and 28 (varies with which countries get sampled), growing from 10-28 to 11-06 with 0–3 new per day (daily_all_countries data card).
- China: 10-28 ratings, 7 in 11-06 with 12, the only market with real growth. US: only 6 ratings (iTunes lookup, 11-06). Other countries: 0–4 ratings each (appstore library app_profile).
Charts
- A total of 65 stores have charted in the Medical category (appstore library app_day, 10-28 through 11-06). Best ranks: Panama #4, Slovenia #4, Ecuador #6, Egypt #7, Iceland #13, Singapore #20. All of these stores have 0 ratings.
- Panama's trajectory: from 10-28 to 11-03 it sat between #52 and #184 on the Medical Top Free chart; on 11-05 it suddenly entered Medical Top Grossing at #4,11-06, then #8 (chart_rank). An app with 0 ratings jumping to #4 overnight likely means a handful of subscriptions, which shows how close to zero the Medical Top Grossing bar is in small countries.
- The China store has been on the Medical Top Grossing chart for 10 consecutive days, ranking #35–#86 (mostly #35–#57). 11-06 is at #50, with "Piyo Log" (49), "Discover the Human Body 3D" (51), and AMBOSS (52) above and below it (chart_rank, cn 6020 grossing). This is the only signal of sustained paid revenue.
- It has never charted in the US store; the UK, Germany, and Japan only appeared sporadically, between #63 and #196 on Medical Top Grossing.
Reviews
- All 3 reviews I could collect come from the China store, all 1 stars: "What is this... Worse than the free software I had, what did I even buy!!?" (10-13, v1.2.4), "Clunky to use" "Doesn't work well" (10-25) (appstore library reviews).
- So people in China did pay, but satisfaction after paying is very poor. This is one-off conversion, not retention.
2. Competitors
Searching the appstore library by name for similar apps (app_name containing dicom, PACS, image reading, medical imaging, etc.) turns up 100+ apps. Few have real scale. A large batch launched in the last 2026 year:
| Product | Platform | Launched | Scale (total ratings, appstore library 11-06) | How it differs from MesDICOM |
|---|---|---|---|---|
| IDV - IMAIOS DICOM Viewer | iOS/Android | 2018-12 | iOS about 3,100 (China 815); Play 956k installs | Made by French medical education company IMAIOS; an established brand with good reputation and the category leader. MesDICOM's features are roughly similar (MPR/3D), but it has no brand. |
| Falcon Mx / Falcon MD / Falcon Patients | iOS | 2020-02 / 2024-02 / 2026-10 | Mx about 2,171, has charted in 61 stores | Long-established professional viewer from UK-based iCat; recently split out a patient-facing version |
| Imaging anatomy atlas (Osmanco) | iOS | 2021-02 | China 1,796 | Focused on anatomy learning, with DICOM viewing on the side, aimed at medical students |
| Ossidex: DICOM Viewer CT & MRI (China store name "DICOM阅片 CT 核磁共振 X光片") | iOS | 2026-04 | 84 (China 26) | Closest playbook: indie developer, multilingual localized titles, subscription. Launched 4 months before MesDICOM and has more ratings. |
| DICOM Viewer : MRI CT XRAY & +(Abdullah Aguiouaz) | iOS | 2026-03 | 37 | Same batch of new launches, English only |
| OsiriX HD | iOS | 2011-02 | Has charted in 75 stores | Long-running paid pro software for doctors |
| DICOM Viewer: MRI CT Studio (Murat Sait Dogan), DICOM Vault (ADIOS Yazılım) | iOS | 2026-07 / 2026-05 | 1–2 | Also Turkish developers, launched in the same period |
| DICOM Viewer: DCM & MRI Reader, Slicora, DICOM Viewer: Pro, Priors, Universal Dicom Viewer, Lumen, DICOMpocket, DicomLens (Play 3,245 installs)…… | iOS/Android | 2026-04 to 2026-10 | 0 to a few | A batch that appeared over the past 2026 years, mostly small viewers likely written with AI assistance |
| MicroDicom, Horos, Weasis, RadiAnt | Desktop / open source | Years | Mainstream on desktop | Free desktop software. The TikTok videos teaching "how to view the disc the hospital gave you" mostly promote MicroDicom. |
| Hospital imaging cloud / scan-to-view | Big-company features | — | Already widespread in many Chinese hospitals | More and more Chinese patients scan a code and view scans directly in WeChat, replacing discs |
Conclusion: this is not an empty niche. In 2026 years a dozen-plus new products have arrived for the same need, and MesDICOM is not the leader among them (Ossidex is earlier and has more ratings).
3. Where users come from
| Channel | Evidence | Confirmed? |
|---|---|---|
| App Store search + localized titles (ASO) | 13 languages; the China store title is written directly as "DICOM医学影像查看器" with the keyword up front; the China store is the main source of ratings and paid conversions | Inferred (very likely the main channel) |
| System file association | The description says it supports "Open in MesDICOM" from the Files app, Safari, and AirDrop, and can recognize DICOM files without an extension. When a patient gets a disc copy or ZIP, iOS lists it as an option for opening the file. | The product mechanism is confirmed; how much traffic it brings is unverified |
| Cross-promotion across the developer's portfolio | onnex.co lists 12 apps, all in the "XX Viewer" style (PDF on Watch, STL/3D models, DICOM, ePUB) | Unverified whether the apps cross-promote inside the app |
| Paid ads | The Meta ad library, TikTok ad library, and Google Ads Transparency Center (our libraries) show no ONNEX/MesDICOM; however, these libraries only track products on our list | Not found ≠ not advertised |
| Social media and press | X (x-radar, 90 days): "MesDICOM" 0 posts; TikTok search for MesDICOM returns no relevant videos; Jina/Brave search only returns store pages and onnex.co, with no press, Reddit, or forum posts | Confirmed to be essentially zero |
| Google Play | 2026-09-23 has com.onnex.mesdicom, with 1,331 installs (googleplay library) | Confirmed, small scale |
4. Why it has got this far (the reasons)
- A one-off need, but a scattered one: patients get CT/MRI discs or ZIPs from hospitals and have no computer at hand. People who search "DICOM" in the App Store have a very clear goal and are willing to pay for a weekly subscription to see the images right away. The line "after I bought it..." in the China reviews is exactly this impulse purchase.
- Low bar for the Medical grossing chart: in many small countries, a few subscriptions are enough to reach the top 10 of the Medical Top Grossing chart (Panama ranked #0 with 4 ratings). So "charted in 30–40 countries" is mostly a product of how the charts work, not real volume.
- Factory-style reuse: ONNEX had already built Mes3D (STL/3D model viewer, 2026-04) and WristDocs (PDF viewer on Watch, 399 ratings). The 3D rendering, file import, and subscription paywall were ready to go, so switching them to DICOM got it live quickly, with 13 languages launched together.
- Chinese localization done early: the China store title and keywords are all in Chinese, matched to the scenario of Chinese patients who get a disc and don't know how to view it. The China store became the only market that has consistently stayed on the grossing chart.
5. Differentiation
- The functionality (MPR, 3D volume rendering, measurement, DICOMweb, PDF export, all processing on-device) is genuinely more complete than most small viewers launched in the past 2026 years, close to IDV. But users can't tell that from the store page, and it hasn't translated into word of mouth: all 3 reviews are 1 stars.
- The real difference is distribution, not the product: localized titles in 13 languages plus iOS file associations. That's why it picks up scattered revenue across several countries, but it's easy to copy, and a dozen similar apps already are (Ossidex's Chinese name, 「DICOM阅片 CT 核磁共振 X光片」, is the same playbook).
- So the differentiation is weak, and it doesn't explain any real growth.
6. How many people know about it
Few people know about it yet: no TrustMRR listing, the founder hasn't posted revenue, 0 mentions on X, no press coverage. But the 「DICOM viewer」 category itself is already on many small developers' radar (a dozen-plus new entries in 2026 years).
7. What others can learn
- The demand is 「a patient gets a specialist file format they can't open」 (DICOM, STL, DWG, HEIC, EPUB, and so on). Build a viewer, stuff the title with format keywords, localize it into several languages, and register as a default handler to pick up the long-tail store search traffic.
- The subscription paywall sits at the first file open, relying on impulse payments. The cost is bad reviews and refunds, and medical-category apps also carry review risk.
- When looking at real paying users, only look at large countries or storefronts with a high enough rating threshold (e.g. China's medical Top Grossing, ranks 35–60), not small-country charts.
8. Still unclear
- How much daily revenue corresponds to China medical Top Grossing ranks 35–50 is unverified (estimated in the tens of dollars per day).
- Whether they run Apple Search Ads (App Store search ads, we have no data) is unverified.
- Subscription retention and refund rates are unknown; the review sample is only 3 reviews.
- Whether the actual price tiers by country (weekly subscription at $3.99 or $5.99) were A/B tested per storefront is unverified.
